Dementia Care Management 101

When a loved one is living with Alzheimer’s disease or another form of dementia, it’s important to know what to expect and how to provide the level of care your loved one needs. If you and your family members are caring for your loved one together, or you are hiring someone to assist you, it’s crucial to come up with a care management plan to ensure your loved one’s needs and wishes are met as they desire. “When more than one person is caring for a loved one with memory loss, it can leave some room for miscommunication. Managing a care plan together while being on the same page about who does what and when can help to make the caregiving process more seamless,” says Jillian Castellano, Community Relations Director at YourLife™ of Coconut Creek, a Memory Care community in Coconut Creek, Florida. “At our community, our care teams collaborate, connect and create a care plan that is best suited to the needs of the person with dementia. This helps to ensure everyone knows what role they play in the care the person receives and cutting down on miscommunication, mishaps and more. In fact, it creates a stronger team and the optimal balance of care with less burnout and caregiver stress. ”

Dementia Care Management: Enhancing Your Loved One’s Life

Whether you are caring for your loved one at home with the help of friends and family or a hired professional, these techniques can ensure smooth management of care and more inspiring days for your loved one. Consider some of the following ways to make dementia care management run smoother.

  • Sit down and discuss tasks. What is your role in caregiving and what are the others supposed to do? Will you take on primary care and delegate tasks to everyone else? Will they assist each day, every other or for simply a few hours? Knowing this and getting it in writing ahead of time can help to make sure everyone knows what is going on. Does your loved one’s prescription need picked up weekly? Assign that task to someone who will drive by the pharmacy. Make it easier for everyone by finding simple, clear ways to help.
  • Check in. Are you off for the day as someone else cares for your loved one? Check in to see how things are going and ask if they need anything. Let the care team know you’re available to talk to if there are any questions or concerns.
  • Designate someone to create a plan for the future. Did your loved one let you know what their care preferences are for the future? What happens if you can no longer care for them at home? If you have a loved one who lives further away, or one who can’t designate a lot of time to caregiving, ask them to research and tour memory care communities. This can take up a big chunk of a caregiver’s time, so allowing someone else to do this can not only help someone to feel useful during this journey, but it also takes something off your plate.
  • Realize that your plans could always change. Caregiving is unpredictable. What works one day may not work the next. This can be frustrating, but with a good support system and loved ones by your side ready and willing to help, it can be made easier. Simply adjust your care plans and see what works.
